Participant Release & Waiver of Liability
By enrolling in and participating in any class, program, rehearsal, workshop, performance, event, or activity offered by Cam Dance Company, I acknowledge and agree to the following terms and conditions.
If the participant is under the age of 18, this agreement must be completed and accepted by a parent or legal guardian on the participant’s behalf.
Voluntary Participation
I acknowledge that my participation (or my child’s participation) in activities at Cam Dance Company is voluntary. I confirm that I (or my child) am physically able to participate in dance-related activities and have no medical condition that would prevent safe participation, except as disclosed to Cam Dance Company in writing.
Assumption of Risk
I understand that dance instruction and related activities involve inherent physical risks. These risks may include, but are not limited to, slips, falls, collisions, sprains, strains, muscle soreness, joint injuries, fractures, or other physical injuries. I understand that not all risks can be foreseen.
I knowingly and voluntarily assume full responsibility for any and all risks of injury, illness, or damage that may arise from participation in Cam Dance Company activities, whether occurring on or off the premises.
Release and Waiver of Liability
In consideration of being allowed to participate in activities at Cam Dance Company, I hereby release, waive, and discharge Cam Dance Company, its owners, instructors, employees, contractors, volunteers, and affiliates from any and all claims, demands, actions, or causes of action of any kind arising out of or related to participation in any Cam Dance Company activities.
This release includes, but is not limited to, claims for personal injury, bodily injury, property damage, emotional distress, or wrongful death, whether caused by negligence or otherwise, to the fullest extent permitted by law.
This release is binding upon myself, my heirs, successors, assigns, and personal representatives.
Instructional Touch Acknowledgment
I understand that dance instruction may involve physical guidance for educational purposes. Instructors may use appropriate physical contact to assist with posture, alignment, balance, or movement technique. I acknowledge that such contact is a common and accepted part of dance training.
I understand that it is my responsibility, or the responsibility of the parent/guardian, to communicate any boundaries or concerns regarding physical contact to Cam Dance Company staff.
Medical Treatment Authorization
I understand that Cam Dance Company does not provide medical, health, dental, or disability insurance for participants.
In the event of an emergency, I authorize Cam Dance Company to seek reasonable medical treatment for myself or my child, including first aid, emergency transportation, or treatment by medical professionals if deemed necessary.
I agree to be fully responsible for any medical or dental expenses incurred as a result of participation in Cam Dance Company activities, regardless of insurance coverage.
I release Cam Dance Company from any liability arising from medical care or decisions made in good faith during an emergency.
Insurance Responsibility
I acknowledge that Cam Dance Company does not carry insurance coverage for participants. I accept full financial responsibility for any medical expenses related to injuries or conditions resulting from participation in Cam Dance Company activities.
Photo and Video Release
I grant Cam Dance Company permission to photograph and/or record video of myself or my child during classes, rehearsals, performances, or events. I understand that these images or recordings may be used for promotional, educational, or marketing purposes, including on websites, social media, print materials, or other media, without compensation.
Rules and Policies
I agree to comply with all rules, policies, codes of conduct, and safety guidelines established by Cam Dance Company. I understand that failure to follow studio policies may result in removal from classes or activities without refund.
Any questions or concerns regarding policies or decisions should be directed promptly to Cam Dance Company management.
Acknowledgment and Agreement
I certify that I have read and fully understand CAM Dance Company Participant Release & Waiver of Liability. I acknowledge that by accepting this agreement, I am waiving certain legal rights. I voluntarily agree to all terms as a condition of participation in Cam Dance Company activities.
If the participant is a minor, I certify that I am the parent or legal guardian and have full authority to sign this agreement on their behalf.
